APM Assessment Services is seeking an Aged Care Clinical Assessor for a full-time, onsite position in Albany. The salary offered is $98K plus super plus travel reimbursement.

In this role, you will be central to identifying the support needs of older adults and connecting them with services to foster independent living and a high quality of life. Daily travel across your designated service area is an essential component of this position to meet operational and client requirements, providing assistance where it is most needed.

Your Responsibilities:

• Conduct comprehensive Aged Care Assessments to ascertain the needs of older adults.
• Facilitate referrals to essential services that support independent living.
• Actively promote the maintenance and optimisation of functional capacity in daily activities.
• Engage collaboratively with families and carers, acknowledging their role as key stakeholders.
• Ensure all assessments adhere to the Department of Health's contractual obligations.

What You Bring:

• A minimum of one year of experience working with older Australians, which includes client assessments and/or service delivery.
• Tertiary qualifications in a health-related discipline, coupled with professional registration with a relevant association (e.g., AHPRA).
• Proficiency in Microsoft Word, Excel, Outlook, and client management systems.
• A current Australian driver’s licence, a comprehensively insured vehicle, and the capacity to travel within your service area.
