APM Assessment Services is seeking a full-time Aged Care Clinical Assessor to join our team in Rockhampton. This onsite role offers a salary of $98K plus super plus travel reimbursement.

As a Clinical Aged Care Assessor, your primary responsibility will be to identify the needs of older adults and connect them with services that support their independence and quality of life. Daily travel throughout your designated service area is a core requirement, involving in-home assessments and direct client engagement within the community to understand individual requirements and provide necessary support.

Your Role:

• Conduct thorough Aged Care Assessments to identify the needs of older adults.
• Facilitate referrals to essential services that support independent living.
• Actively promote the retention and maximization of function and capability in daily living.
• Work closely with families and carers, recognizing them as key stakeholders.
• Ensure all assessments are conducted in line with Department of Health contractual requirements.

What You Bring:

• At least 1 year working with older Australians, including client assessments and/or service delivery.
• Tertiary education in a health-related discipline together with professional registration with a relevant association (e.g. AHPRA).
• Proficiency in Word, Excel, Outlook, and client management systems.
• Current Australian driver’s licence, a comprehensively insured vehicle, and the ability to travel within your service area.
