Dhelkaya Health is seeking a skilled and compassionate At Home Nurse to join our dynamic At Home Program. This casual, onsite role is based in Castlemaine.

The At Home Program encompasses Hospital in the Home (HITH), Geriatric Evaluation Management (GEM) at Home, Residential In Reach (RIR), and Rehabilitation in the Home (RITH). Operating seven days a week, the program offers a safe and effective alternative to hospital-based care through coordinated, multidisciplinary service delivery.

Key Responsibilities:

• Deliver expert, client-centred nursing care in home settings.
• Develop and implement individualised care plans.
• Advocate for clients and support their health goals.
• Provide education to clients and carers to promote independence and wellbeing.
• Monitor and respond to clinical deterioration and changing needs.
• Collaborate with multidisciplinary teams to ensure coordinated, high-quality care.
• Support transitions of care, including discharge planning and referrals to other services.

You will be actively involved across all phases of care, ensuring clients receive the right level of support at every stage.

About You: We are looking for a nurse who:

• Demonstrates strong clinical assessment and decision-making skills.
• Has experience or interest in community, aged care, or acute care services.
• Is passionate about delivering patient-centred care in a home environment.
• Communicates effectively with clients, families, and multidisciplinary teams.
• Is adaptable, organised, and able to work independently.

Role Details:

• Classification: YU15
• Salary per hour: $58.34 + casual loading + superannuation + generous salary packaging option.
• Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028.
