Respect Group Limited is seeking a dedicated Care Coordinator to join its regional aged care team on a full-time, onsite basis in Glenella. This role involves close collaboration with the Care Manager to coordinate person-centred care, support clinical excellence, and ensure residents receive safe, high-quality care. The position also includes guiding and supporting care staff and contributing to a positive and compassionate care environment.

About the Role The Care Coordinator is responsible for coordinating residents’ person-centred clinical care in alignment with policies, procedures, and professional standards, in collaboration with the Care Manager. The primary purpose is to focus on individual resident needs, ensuring they have a current documented Care Plan that meets their requirements and is reviewed and delivered by the multi-disciplinary care team.

Key responsibilities include:

  • Coordinating resident care with the Care Manager, encompassing admissions and assessments.
  • Developing and reviewing person-centred care plans with the care team and families.
  • Providing and overseeing clinical care, including Activities of Daily Living (ADLs), medications, wound care, and observations.
  • Maintaining accurate resident documentation, such as care plans, charts, and incident records.
  • Monitoring resident condition and escalating changes, including falls, wounds, and deterioration.
  • Supporting infection control measures and implementing risk reduction strategies.
  • Supervising and supporting care staff through allocation, guidance, and training.
  • Ensuring duty of care by adhering to policies, responding to incidents, and advocating for residents.

About You To thrive in this role, you will possess:

  • A Bachelor of Nursing.
  • Current Australian Health Practitioner Regulation Agency (AHPRA) registration.
  • An understanding of the aged care accreditation process.
  • Experience working cooperatively with multi-disciplinary care teams.
  • Strong clinical assessment, documentation, and practice skills.
  • Effective communication and interpersonal skills.
  • Proficient computer and information management skills, including the ability to use Microsoft and clinical software systems.
