Coastal Home Care is a rapidly expanding organisation in Australia focused on providing premium, dignified, and joyful home care experiences. We deliver bespoke, concierge-level support, moving away from standardised care. We are seeking a passionate Care Manager with a Nursing Qualification (RN) to join our dynamic team.

Role Overview

Reporting to the Service Manager, your primary role is to champion client safety, independence, and happiness within their homes. This is a hybrid role, with work from home and one day per week in the Buderim office.

Responsibilities:

• Care Planning: Develop, review, and tailor comprehensive Care Plans in close consultation with clients and their families.
• Service Oversight: Collaborate with the rostering team and third-party suppliers to ensure appropriate support worker allocation and scheduling to meet client goals.
• Financial Stewardship: Monitor individual client budgets to ensure care plans are accurately funded and sustainable.

About You

You are empathetic, highly organised, and driven by the desire to make a tangible difference in the lives of older Australians. You are comfortable working autonomously while also valuing the camaraderie of a fun, dynamic team culture.

Key Selection Criteria:

• Ideally, 12+ months of experience in Home Care, Community Aged Services, or Care Management.
• Experience across Social Services, Aged Care, Disability, or the broader Health Industry. Registered/Enrolled Nurses are strongly encouraged to apply.
• Exceptional time management, strong computer/administrative literacy, and top-tier communication skills.
• A professional, empathetic approach with a proven ability to exceed customer expectations.

Logistics & Requirements:

• A valid Australian driver’s licence and a safe driving record.
• Your own reliable vehicle and a willingness to travel to client homes.
