Department of Health - Queensland is seeking a full-time Chronic Disease Management Team Lead (Multi-Stream) for a remote role based in Cairns.

This leadership position involves overseeing chronic disease programs across 11 Cape York communities, aiming to enhance health outcomes and quality of life.

About the Role:

This is a multi-stream leadership opportunity dedicated to improving prevention, management, and health outcomes for chronic diseases in some of Queensland's most remote areas.

• For Health Practitioners:
• Lead and manage multidisciplinary clinical programs focused on primary, secondary, and tertiary chronic disease prevention and management across 11 Cape York communities.
• Oversee advanced diabetes education, Nurse Practitioner Chronic Disease services, and the Cape York Kidney Team.
• Services are delivered via outreach clinics, telehealth, and community-based care, in partnership with Aboriginal Health Workers and local health services.
• A strong emphasis is placed on culturally safe care, chronic disease strategies, health promotion, and performance monitoring.

• For Nurses:
• Lead clinical teams delivering health promotion and chronic disease management at a primary health care level across Cape York communities.
• The portfolio includes diabetes, kidney care, and chronic disease programs.
• Focus on healthy lifestyle initiatives such as nutrition, physical activity, prevention, and self-management support.

What You'll Receive:

• Base Salary: HP5 $144,991 – $151,289 p.a. OR NRG7 $139,951 – $151,949 p.a.
• Workplace Benefits: Provided by Torres and Cape Hospital and Health Service.
• 12.75% employer superannuation contribution.
• 17.5% annual leave loading.
