Company: BlueCare Location: Innisfail & Surrounding Areas Work Type: Full-time On-site/Remote: On-site Salary: $45.29 – $57.06 per hour

BlueCare is seeking a Clinical Care Partner to join their team in Innisfail. This full-time, on-site position offers a remuneration package of $45.29 - $57.06 per hour, plus superannuation, not-for-profit salary packaging to increase take-home pay, allowances, travel reimbursement (kilometres), and employee discounts. Classification will be QRN1.1 – QRN1.7, determined by demonstrated clinical hours and experience.

As a Clinical Care Partner, your primary responsibility is to ensure clients receive person-centred, high-quality care. This involves overseeing and coordinating complex clinical and care needs through assessment, care planning, and service coordination, with the aim of achieving optimal care outcomes for each client from their Aged Care Service funding.

Key Responsibilities:

  • Develop and review complex care plans in collaboration with the client and the multidisciplinary care team.
  • Manage budgets and facilitate multidisciplinary care activities.
  • Liaise with clinical teams to monitor and respond to the evolving needs of complex community clients.
  • Continuously monitor, review, and evaluate care plans, services, and outcomes.
  • Assist clients in navigating the care system and connect them with necessary resources when required.

Role Requirements:

  • A Registered Nurse or Allied Health professional holding current AHPRA registration and unlimited work entitlements is essential.
  • Demonstrated understanding and sound knowledge of client-centred, community-based care service delivery.
  • Demonstrated experience and knowledge in Community Funding streams and programs.
  • Excellent communication and clinical problem-solving skills.
  • A current Australian Driver’s Licence is essential.
  • A roadworthy and insured vehicle (Third Party Property minimum) for work use is essential.
  • Confidence in managing your time effectively, including an ability to work autonomously.
