Community Registered Nurse (Health Lead)

Company: Ability WA Location: Coolbinia Work Type: Part-time (22.5 hours per week) Salary: $38.57 - $48.82 per hour, plus superannuation and salary packaging benefits (up to $18,550) Reporting To: Nurse Manager

Ability WA is seeking an experienced Registered Nurse for an internal opportunity as a Health Lead. This role supports the health and wellbeing of Ability WA customers and provides clinical leadership to Community Services teams. It offers a combination of direct clinical practice, case management, coaching, and clinical governance, contributing to both immediate impact and strategic development.

The Opportunity

As a Registered Nurse in the Health Lead team, you will assist the Clinical Nursing team in addressing the health needs of customers with disability-related and complex health requirements. You will provide clinical advice and consultancy, primarily through telepractice, with direct nursing support as needed. The role includes ongoing practice of key clinical skills such as PEG management, wound care, and catheter management. Dedicated office-based time is allocated for care plan development, case management tasks, and contributing to clinical frameworks.

This is a part-time, onsite role based at our Coolbinia office, with flexible working arrangements available.

Key Responsibilities

In this position, you will:

  • Provide clinical advice, support, and interventions to customers with disability-related health needs.
  • Monitor and support customers presenting with complex or high-risk health conditions.
  • Coach and support Community Services teams, Managers, Coordinators, and Support Workers.
  • Collaborate with health professionals, therapists, families, and guardians to achieve customer outcomes.
  • Develop, review, and implement clinical assessments and support plans, such as bowel care plans.
  • Support the Nurse Manager and Quality & Governance team with clinical practice and governance frameworks.
  • Review and provide advice on clinical incidents, hospital admissions/discharges, medication management, AMRs, and audits.

What We Are Looking For

You will possess:

  • Current AHPRA registration as a Registered Nurse.
  • A minimum of two years' Registered Nurse experience.
  • Strong contemporary clinical skills in supporting customers with complex needs.
  • A coaching, mentoring, and collaborative approach.
  • Strong communication and stakeholder engagement skills.
  • Alignment with Ability WA values.

You will also hold, or be willing to maintain:

  • National Police Clearance (obtained within the last 6 months).
  • Driver’s Licence.
  • NDIS Worker Screening Check and NDIS Worker Orientation Module.
  • Current First Aid Certificate (HLTAID010 or HLTAID003 minimum).
