SA Health - Eyre and Far North Local Health Network

Port Lincoln | Full-time | Onsite

Salary: ENC/END $66,552 - $77,824 p.a.

Eyre and Far North Local Health Network, Country Health Connect - Home Support is seeking a compassionate and motivated Enrolled Nurse to join the Country Health Connect team as a Care Partner in Port Lincoln. This permanent, full-time role focuses on supporting individuals receiving Support at Home and Commonwealth Home Support Program (CHSP) services, aiming to foster their independence, wellbeing, and quality of life within their homes and communities.

About the Role

Working under the direction and supervision of a Registered Nurse, you will contribute to person-centred care delivery through clinical support, care coordination, monitoring, and advocacy.

Key responsibilities include:

  • Delivering safe and effective nursing care within your scope of practice.
  • Supporting participants to maintain their independence at home.
  • Monitoring wellbeing and escalating clinical concerns as appropriate.
  • Assisting with care planning and service coordination.
  • Maintaining accurate clinical documentation.
  • Supporting participants, carers, and families through education and guidance.

About You

To be successful in this role, you will demonstrate:

  • Enrolled Nurse registration with the Nursing and Midwifery Board of Australia.
  • A minimum of 3 years of post-registration experience.
  • Strong communication and teamwork skills.
  • A commitment to person-centred care and community wellbeing.
  • The ability to work independently and within multidisciplinary teams.
  • A current driver’s licence and a willingness to travel.
