The Bolton Clarke Residential group encompasses 88 Aged Care Homes across QLD, NSW, VIC, SA & WA, including the Allity, McKenzie & Acacia Living Group.

About the role The Care Partner functions as the designated care manager under the Support at Home Program. This role is responsible for undertaking care management activities that support older individuals to maintain safe and independent living at home. You will be instrumental in planning and supervising care service delivery across all eligible ongoing support classifications, and will collaborate with clinical team members when participant needs necessitate clinical input or complex care planning.

As a Care Partner, your responsibilities will include:

  • Collaborating with participants to develop individualised care plans, integrating their goals, service preferences, cultural needs, risk factors, and AT-HM requirements.
  • Liaising with general practitioners, specialists, and allied health providers to facilitate integrated care.
  • Reviewing care notes and service delivery data to proactively identify and address emerging risks or changes in participant needs.
  • Consistently meeting quality compliance and audit requirements, ensuring accurate and up-to-date records are maintained.
  • Facilitating or participating in case conferences to ensure care plans reflect shared decision-making and supported risk-taking.
  • Monitoring spending to prevent underspends and overspends, and aligning resource allocation with evolving needs.
  • Maintaining accurate, timely care notes, ensuring documentation supports outcomes under the Strengthened Aged Care Quality Standards.
  • Ensuring care management claims comply with Support at Home business rules, specifically excluding administrative, rostering, or travel tasks.
  • Supporting participants (and their carer or supporter) in making informed service decisions.

To be successful, you will have:

  • A minimum Cert IV qualification in aged care, or a Diploma-level qualification in Nursing, Community Services, Aged Care, Case Management, or equivalent experience.
  • Demonstrated experience in care coordination, case management, or similar roles within aged care, health, or disability services.
  • A strong understanding of the Support at Home Program, Quality Standards, and aged care rights.
  • Strong communication, empathy, and cultural competence.
