Care Partner/Case Manager - Support at Home (Aged Care)

Coburg Aged Care Pty Ltd is seeking a Care Partner/Case Manager to join its team in Tullamarine. This full-time, remote position offers a salary of $80,000 – $95,000 per year.

About Esse Care Esse Care is a registered aged care provider dedicated to delivering high-quality, person-centred care to older Australians within their own residences. Through the Support at Home program, the organisation assists clients in maintaining independent, safe, and fulfilling lives on their own terms, in their preferred environment. The organisation is expanding and is looking for a compassionate and experienced Case Manager/Care Partner to join its close-knit team supporting older Australians across Melbourne.

The Opportunity This is a client-facing role focused on care coordination and the management of individual support plans, aiming to positively impact the lives of those supported. The position operates within a close-knit, supportive team structure with a flat hierarchy, encouraging autonomous work while ensuring clients receive timely and appropriate services.

Key Responsibilities

  • Cultivate strong, respectful relationships with clients, their families, support workers, and external providers.
  • Coordinate personalised support services aligned with clients’ objectives and health statuses.
  • Undertake assessments, and develop and review care plans for each client.
  • Make decisions that prioritise client safety, wellbeing, and quality of life.
  • Lead and supervise a team of Support Workers, ensuring the quality and continuity of services.
  • Monitor and manage client budgets to ensure full and appropriate utilisation.
  • Maintain adherence to current legislation, funding guidelines, and internal policies.
  • Collaborate and communicate with the client’s support network.
  • Document precise and timely progress notes concerning client interactions, changes in condition, incidents, and follow-up actions.
  • Review progress notes submitted by support workers, suppliers, and allied health professionals to ensure coordinated, high-quality care.
  • Manage the admission of new clients, including intake assessments and initial meetings.

About You

  • Demonstrated experience in aged care, home care, community services, or a health-related setting.
  • Proficiency in assessment, care planning, and case management skills, with the capacity to coordinate safe, goal-aligned support.
  • Confidence in working independently and managing multiple priorities.
  • Excellent communication, relationship-building, and leadership capabilities.
  • Working knowledge of My Aged Care and the Aged Care Quality Standards.
  • Possession of a current National Police Check, or a willingness to obtain one.
  • A driver's licence, an owned vehicle, and a willingness to travel across Melbourne.
  • Bilingual or multilingual ability is a strong advantage, particularly Turkish, Arabic, or other community languages spoken by clients.
  • A clinical background, such as nursing or allied health, is a bonus but not required.
