Latrobe Community Health Service is seeking a self-motivated and experienced Care Partner – Duty to join the Support at Home team. This is a varied and rewarding role that provides essential coverage during planned and unplanned Care Partner leave, ensuring older people continue to receive safe, responsive, and consistent support to live independently at home.

This role operates across a diverse client group receiving services through the Support at Home program. You will step into active caseloads when required, respond to client and staff queries, and ensure continuity of care, service coordination, and case management responsibilities.

About the Role

In this position, you will:

• Provide responsive support to clients, representatives, staff, and service providers.
• Cover Care Partner caseloads during scheduled and unscheduled leave.
• Undertake assessments, reassessments, care planning, monitoring, and reviews.
• Identify risks and support practical risk mitigation strategies.
• Promote client choice, wellness, reablement, and goal-directed care.
• Monitor client budgets and ensure care plans align with available funding and program guidelines.
• Respond to escalated client care matters and take prompt action where needed.
• Liaise with internal teams, health professionals, community services, and external providers.
• Maintain accurate records, case notes, and documentation.
• Support continuity of service delivery so clients can remain safely at home for longer.

Each day will bring variety, with potential tasks including responding to urgent client care matters, completing assessments and reviews, updating care plans, monitoring budgets, managing risk, liaising with service providers, and working with clients and families to make informed decisions about their care. As a member of a larger team, you will oversee a portfolio of clients, ensuring the necessary support is provided for them to safely stay at home while working towards their goals.

About You

To be successful in this role, you will possess strong case management experience, excellent communication skills, and the ability to work confidently across a varied and changing workload. You will be someone who can think critically, problem-solve, prioritise competing demands, and build positive relationships with clients, families, carers, colleagues, and service providers.

You will also have:

• Experience working in a community setting.
• Strong assessment, care planning, and client support skills.
• An understanding of the aged care system, Support at Home, client-directed care, and the Aged Care Standards.
• Good organisational and time management skills.
• Confidence using technology, databases, and written documentation systems.
• The ability to monitor client budgets and package expenditure.
• A calm, practical, and compassionate approach when responding to client concerns.
• A commitment to excellent customer service and positive client outcomes.
• A Victorian driver’s licence.

A relevant qualification in case management, community services, nursing, aged care, allied health, or a related field is required.
