Care Partner (Experienced)

Progressive Home Care Pty Ltd is seeking an experienced Care Partner for an onsite, full-time role in Beckenham. This position offers an hourly rate of $38.50 – $43.

Responsibilities:

  • Client Support & Care Coordination:
  • Cultivate positive and respectful connections with clients, their families, and carers.
  • Undertake non-clinical client assessments and conduct routine wellbeing checks.
  • Create and uphold personalised support plans that align with client aspirations and preferences.
  • Orchestrate and schedule home care services, support workers, and community-based supports.
  • Track client satisfaction and promptly address any concerns or service adjustments.
  • Aid clients in navigating available community, aged care, and disability support options.
  • Service Delivery & Administration:
  • Maintain precise client records, progress notes, and service documentation.
  • Ensure services are delivered in adherence to organisational policies and procedures.
  • Facilitate the onboarding of new clients and support service commencement procedures.
  • Collaborate with rostering and operational teams to ensure uninterrupted care and support.
  • Assist with incident reporting and subsequent actions as may be required.
  • Communication & Relationship Management:
  • Serve as a primary point of contact for clients and their families.
  • Liaise with support workers, coordinators, external service providers, and community groups.
  • Foster a positive client experience through responsive and empathetic communication.
  • Quality & Compliance:
  • Contribute to compliance with:
  • Aged Care Quality Standards
  • Work Health and Safety requirements
  • Organisational policies and procedures
  • Engage in continuous improvement efforts and feedback mechanisms.

Qualifications:

  • Current Criminal Record Check
  • First Aid certificate and CPR
  • Current Australian Driving License

Competencies and Experience:

  • Demonstrated experience in home care, aged care, community care, disability services, or care coordination.
  • An understanding of:
  • Home Care Packages (HCP)
  • Support at Home Program
  • Experience utilising care management or rostering systems.
  • Knowledge of culturally safe and inclusive care practices.
  • Experience working with culturally diverse communities, including Aboriginal and Torres Strait Islander peoples, older persons, people with disability, or vulnerable individuals.

Desirable:

  • Experience in assessment and care planning.
  • Experience in Dementia and / or Palliative care.
  • Knowledge of Aged Care Quality and Safety Standards.
  • Familiarity with legislation and regulations pertinent to aged care.
