Southern Cross Care (Qld) Ltd is seeking a part-time Care Partner for an onsite role in Ipswich.

Join a team dedicated to making a significant impact for both clients and colleagues. Southern Cross Care QLD presents an exciting opportunity for a proactive and self-directed part-time Care Partner to join our Home Care team based in Raceview.

As a Care Partner, you will champion Aged Care clients living at home across the Ipswich and West Moreton region, ensuring excellent service delivery and high-quality care.

About the Role:

Based in Raceview, this dynamic role involves managing a caseload of Support at Home and Commonwealth Home Support Program (CHSP) clients, with a focus on achieving exceptional care outcomes. You will be instrumental in coordinating care strategies, utilising your in-depth knowledge and skills to positively impact clients' lives, ensuring each client feels 'Known and Loved' by our service.

Key Responsibilities:

  • Develop personalised care plans in collaboration with clients and their families, where appropriate.
  • Develop budgets with clients, effectively utilising funding to meet their needs.
  • Plan and coordinate the delivery of scheduled services, including personal care, meal preparation, cleaning, and transportation.
  • Monitor the implementation of care plans and make adjustments as required.
  • Monitor client funding, taking corrective action to ensure services are provided within budget and supporting the management of any unspent funds.
  • Ensure CHSP services are delivered in accordance with CHSP guidelines.
  • Promote clients' rights, preferences, and independent decision-making.
  • Conduct regular reviews and reassessments to ensure client needs are effectively met.
  • Provide information and support to clients and their families regarding available services and resources.
  • Liaise with healthcare professionals, service providers, and community organisations to ensure coordinated care.
  • Maintain accurate records of Care Management services and document client interactions and service provision.

What You Will Bring:

  • Certificate IV in Community Services or equivalent.
  • Experience in a similar role within the Aged Care or Disability Services Sector.
  • Demonstrated knowledge and experience in delivering services under Support at Home (formerly Home Care Package Program).
  • Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP).
  • Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation.
  • A practical approach with a realistic and sensible mindset.
  • High attention to detail, ensuring accuracy and thoroughness.
  • Ability to follow processes and to create and implement efficient work practices.
  • Ability to manage time efficiently and prioritise responsibilities.
  • Excellent communication skills at all levels.
