Right at Home ACT Canberra is seeking a qualified and dedicated Care Partner for a full-time, onsite role within its commercial home care services. This position is based in the Inner South and Southern suburbs of Canberra.

Reporting directly to the Clinical Manager, the Care Partner will be instrumental in the success of the home care operations. The position allows for the application of skills and judgment both in direct client interaction and in office-based tasks, with potential for future career advancement.

A valid driver's licence is a requirement for this role. Australian work rights are also necessary.

Responsibilities:

• Cultivate strong relationships with clients and their families.
• Allocate staff to clients, considering staff qualifications, availability, and client preferences.
• Coordinate client support, monitor progress, and report on outcomes.
• Refer clients to external agencies for supplementary assistance.
• Evaluate client needs and identify available health, welfare, and other services.
• Address client, staff, and management enquiries and requests promptly via email and other communication channels.
• Maintain precise records of incidents and feedback as needed.
• Adapt work priorities to suit the demands of a fast-paced environment.
• Assist all clients in achieving their personal goals.
• Undertake Client Care Assessments and develop Care Plans.
• Provide clinical direction to team members concerning client management.
• Discuss and promote Right at Home services with medical professionals and referral partners.
• Demonstrate the ability to work effectively in a fast-paced, dynamic setting.

Requirements:

• Certificate III/IV in Individual Support/Disability or a Nursing qualification.
• A valid driver's licence.
• Australian work rights.

Preferred:

• Familiarity with or prior experience in the aged care sector.
• A detailed understanding of the SCHADS Award and Fair Work Requirements is highly desirable.
• Previous experience with Visual Care and/or other rostering systems.
• Understanding and experience with government-funded Home Care Packages.
• A willingness to learn on the job.
