Care Partner Support at Home Community Services - South

Resthaven Inc. is offering two permanent, full-time positions for Care Partner Relievers within its Community Services team. These roles are based at Resthaven Marion Community Services in Marion and Resthaven Onkaparinga Community Services in Noarlunga.

This is an opportunity for an experienced Care Partner or Case Manager seeking a dynamic role focused on making a tangible impact. As a Care Partner Reliever, you will contribute to the Support at Home program by providing supplementary assistance across various care portfolios. Your responsibilities will include ensuring the consistent delivery of high-quality, person-centred care, overseeing and monitoring services, evaluating progress against individual care plans, and collaborating with older individuals, their families, and the wider care team. The aim is to support clients in maintaining safe and independent living within their own homes.

Essential Criteria:

• Experience in community aged care, care coordination, case management, or a comparable client-focused position.
• Experience in program coordination, team leadership, or case management.
• Empathy and understanding of the challenges faced by older individuals, their families, and carers.
• Knowledge and experience with quality systems for evidence-based care, alongside proficient written and verbal communication skills.
• A commitment to supporting older individuals to remain in their homes and communities.
• Demonstrated capacity to operate within a holistic, person-centred model of care.
• The ability to collaborate with individuals to identify their needs, establish personal goals, and develop responsive service packages.
• Proficiency in computer use, including familiarity with MS Office Suite and electronic client care systems.
• A current South Australian driver's license.

Qualifications Highly Recommended:

• Diploma of Nursing
• Certificate III in Individual Support (Ageing) or Health Services Assistance
• Certificate IV in Aged Care, Disability, or Community Services
• Diploma of Community Services (Case Management)
• Diploma of Ageing Studies and Services

Salary: $88,000 - $93,600 base per annum, plus superannuation and salary sacrifice options.

Work Type: Full-time.

Location: Onsite at Resthaven Marion Community Services (Marion) and Resthaven Onkaparinga Community Services (Noarlunga).
