Royal Freemasons Ltd is seeking a dedicated Case Manager to join its Community Services team. This leading not-for-profit organisation is committed to the Relationship Model of Care, prioritising genuine connection and quality service.

About the Role

The Case Manager will provide personalised support to clients living in their own homes, ensuring care plans align with individual needs, preferences, and goals. This full-time position involves 38 hours per week, with flexible hours available.

Reporting to the Manager of Community Services, your responsibilities will include:

  • Leading quality service delivery for your client group.
  • Managing Support at Home budgets effectively and responsibly.
  • Maintaining clear and efficient communication with clients, families, staff, and external stakeholders.
  • Ensuring compliance with all regulatory and accreditation requirements.
  • Delivering services in line with Client Agreements and organisational standards.
  • Collaborating with the team to provide integrated, holistic care.
  • Maintaining up-to-date knowledge and skills relevant to the role.
  • Promoting a positive workplace culture and upholding Royal Freemasons’ Core Values.
  • Completing internal audits and reporting within required timeframes.
  • Operating in a hybrid environment, which includes working from home, visiting clients, and occasional office attendance.

Skills & Experience

Candidates will be considered based on the following:

  • A relevant qualification in Community Services, Health, or Management (essential).
  • A current Victorian Driver’s Licence and a reliable, comprehensively insured vehicle (essential).
  • Experience with Support at Home (highly regarded).
  • Knowledge of the Aged Care Act 2024, Strengthened Aged Care Quality Standards, and community care practices.
  • Competence with MS Office and relevant information systems.
  • Understanding of Consumer Directed Care principles.
  • Strong budgeting, negotiation, and communication skills.
  • A genuine commitment to supporting older people.
  • A current NDIS Worker Screening Check and valid Australian working rights.
