About the Role: As the Clinical Aged Care Assessor, you will contribute to the Footprints Aged Care Assessment Service (ACAS) by conducting holistic aged care assessments. This hybrid clinical position supports responsive, client-centred, comprehensive, fair, and timely assessment pathways for older Australians accessing Commonwealth-funded aged care services. Depending on operational needs, workforce capacity, delegation status, and service demand, you may also undertake Triage Delegate and Assessment Delegate functions.

Your responsibilities will include:

  • Conducting aged care needs assessments.
  • Determining eligibility for assessment services.
  • Reviewing assessment outcomes and support plan recommendations.
  • Exercising delegated authority under the Aged Care Act, adhering to legislative requirements, operational guidance, program expectations, and approved delegation arrangements.

What You Will Bring:

  • Experience working with older individuals and their families/carers, including those experiencing mental illness, disability, homelessness, housing instability, or social and financial disadvantage.
  • Experience in undertaking assessments, triage, care coordination, and goal-oriented support planning within aged care, community, health, or human services settings.
  • Demonstrated ability to apply sound clinical judgment, decision-making, and problem-solving skills within legislative and operational frameworks.
  • Knowledge of aged care reforms, the My Aged Care assessment pathway, and person-centred service delivery principles.
  • High-level computer literacy and experience operating across multiple digital platforms and systems.
  • Ability to establish networks with community aged care providers and regional stakeholders.

Requirements:

  • Tertiary qualification as a Registered Nurse, Occupational Therapist, or Physiotherapist with current AHPRA registration, or as a Social Worker with eligibility for AASW membership.
  • Current QLD Driver's Licence.
  • NDIS Worker Screening (Yellow Card) and Blue Card.
  • Annual influenza vaccination, as per Department of Health and Aged Care requirements.
  • Current First Aid and CPR Certificate (Desirable).
