This contract position offers an opportunity to enhance quality of life outcomes for older individuals in Western Sydney. You will collaborate with multidisciplinary experts across NSW health networks. This role is open to Registered Nurses and Allied Health Professionals.

The role involves assessing, supporting, and empowering older people by creating tailored aged care plans that maximise independence and connect them with necessary services.

Employment Type: Four temporary full-time positions, each for a 12-month contract up to 30th June 2027. Position Classification: Registered Nurse, Allied Health Professionals Level 1/2. Remuneration: $76,364 - $111,427 per annum plus superannuation, dependent on qualifications. Hours Per Week: 38. Job sharing may be considered for part-time hours. Location: Westmead Hospital / Western Sydney Local Health District.

About the Workplace: You will be based at Westmead Hospital, within the Seniors Assessment Service of Western Sydney Local Health District. This service is dedicated to ensuring older individuals access appropriate care at the right time. You will join a skilled multidisciplinary team providing comprehensive aged care assessments in both hospital and community settings.

Key Responsibilities: As a Clinical Aged Care Assessor (ACAT), you will conduct comprehensive assessments to determine eligibility for Commonwealth-funded aged care services. You will also develop tailored support plans to meet the individual needs of older people and their carers.

Your duties will include:

• Completing holistic aged care assessments that consider medical, functional, cognitive, psychosocial, cultural, and environmental needs.
• Developing evidence-based support plans and facilitating access to residential aged care and community services under the Aged Care Act.
• Collaborating with multidisciplinary colleagues and external service providers to coordinate seamless, person-centred care.
• Maintaining accurate clinical documentation and managing assessment data in accordance with legislative and organisational requirements.
• Responding to referrals efficiently, achieving service performance indicators, and upholding high-quality assessment standards.

Essential Criteria:

• Tertiary qualification and current registration with AHPRA in Nursing, Physiotherapy, Occupational Therapy, or a tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Work, or Speech Pathology with eligibility for membership of Speech Pathology Australia.

Selection Criteria:

• Demonstrated post-graduate experience working with older people in an acute or community setting.
• Demonstrated ability to work independently and effectively, encompassing appropriate initiative, organisational, problem-solving, and time management skills.
• Proven written and verbal communication skills, including demonstrated computer literacy.
• Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.
• Understanding or experience in the health and functional support needs of older people and their carers in the community setting.
• Current Drivers Licence (minimum class C) valid within Australia and demonstrated competent driving ability/experience.
