Team National is a multidisciplinary organisation providing disability, aged care, and community nursing services across the ACT and surrounding regions. We support individuals with a wide array of needs, including complex disabilities, chronic health conditions, and high-intensity support requirements. Our integrated service model combines Community Workers, Registered Nurses, Allied Health Professionals, and disability support teams to deliver coordinated, person-centred services aimed at fostering independence, safety, and quality of life. Our participant group includes individuals requiring complex community nursing, respiratory care, Hospital in the Home (HITH), tracheostomy airway management, palliative care, and multidisciplinary clinical coordination.

About the Role We are seeking an experienced Community Worker to join our Support Coordination team. In this role, you will work closely with participants, their families, community nursing teams, allied health professionals, hospitals, government agencies, and disability providers to coordinate services. This coordination will enable participants to safely access the supports they need to achieve their goals. This is an opportunity to contribute to an organisation recognised for supporting participants with some of the community's most complex health and disability support needs.

Key Responsibilities

  • Assist participants in understanding and implementing government-funded support plans.
  • Coordinate disability, aged care, community nursing, and health services.
  • Advocate for participant choice, rights, and independence.
  • Liaise with the NDIA, Support at Home providers, hospitals, allied health professionals, and community organisations.
  • Coordinate multidisciplinary services for participants with complex health and disability support needs.
  • Monitor participant outcomes, service quality, and safeguarding requirements.
  • Maintain participant records, budgets, service agreements, and care documentation.
  • Support participants during hospital discharge, service transitions, and plan reviews.
  • Identify risks and coordinate appropriate responses.

About You To be successful in this role, you will possess:

  • A Certificate IV and/or Diploma Qualification in Community Services, Disability, Social Work, Nursing, or a related discipline.
  • A Qualification in Psychosocial Supports.
  • A minimum of three years' experience within Community Services, Disability, Aged Care, or NDIS Support Coordination.
  • Strong knowledge of the National Disability Insurance Scheme and Aged Care frameworks.
  • Excellent communication and stakeholder engagement skills.
  • Demonstrated ability to coordinate multidisciplinary services.
  • High-level organisational and problem-solving skills.
  • A current Driver Licence.
  • A National Police Check.
  • An NDIS Worker Screening Clearance.
  • Completed NDIS Worker Training Modules.

  • Supported Independent Living (SIL)
  • Positive Behaviour Support
  • Multidisciplinary clinical coordination

Experience or formal training in NDIS High Intensity Support Skills, including tracheostomy airway management, will be highly regarded.
