St Jude's NDIS is seeking a full-time Coordinator - Home & Community to join their team in West Perth. This is an onsite role.

St Jude's Health Care Group, established in 1982, is a respected provider of Aged Care, Allied Health, and Disability Services. Role Summary

The Community Services Coordinator is responsible for ensuring the delivery of optimal, person-centred services that meet client needs. This involves regular engagement with clients, their families, and other stakeholders to foster meaningful, lasting relationships. The role includes managing staffing levels, overseeing the performance of support workers, and accountability for the utilisation of provided funds and adherence to budgetary requirements. Client satisfaction is the primary measure of success.

Responsibilities and Duties

  • Collaborate closely with clients to ensure optimal use and delivery of their funds and services.
  • Confirm adherence to support plans and the maintenance of high-quality progress notes.
  • Manage and support a team of support workers.
  • Ensure services are compliant with standards and that risks are managed effectively.

This position is Permanent-Full Time, with working hours from Monday to Friday, 9am to 5pm.

Skills, Attributes and Experience

The ideal candidate will possess a proven history of delivering quality services within person-centred or community programs. Experience managing participants with complex mental health issues or challenging behaviours is desirable. A solid understanding of the NDIS funding model and associated reporting requirements is necessary to problem-solve and collaborate with stakeholders to resolve complaints. Proactive self-starters who can lead performance appraisals and identify areas for growth and development within a continuous improvement framework are sought.

Key requirements for the role include:

  • Relevant qualifications/certifications or significant experience within the NDIS framework.
  • Excellent documentation and reporting skills.
  • Strong interpersonal skills and the ability to communicate effectively with a diverse range of stakeholders.
  • Must possess or be willing to obtain an NDIS screening clearance, a WWCC, a WA driver's licence, and a reliable vehicle.
