Disability Support Worker – Servant Heart Community Care

About the Role

We are seeking a committed Disability Support Worker for a contract position in Brisbane, QLD. This role involves providing direct support to an individual with Muscular Dystrophy, assisting them in achieving personal goals, maintaining independence, and engaging safely in daily life and community activities. This position is integral to our mission of delivering high-quality, person-centred care and will directly contribute to the life of a vulnerable individual while supporting our commitment to inclusive, community-focused care.

Key Responsibilities

  • Provide personal care and assistance with daily living activities, including hygiene, grooming, meal preparation, and household tasks.
  • Accompany clients to medical appointments, educational programs, and community activities, ensuring their safety and wellbeing.
  • Support clients in building and maintaining social connections, aiming to reduce isolation and encourage meaningful participation.
  • Assist with recreational activities, hobbies, and leisure pursuits aligned with client interests.
  • Maintain accurate, detailed records of support provided, client progress, and any health or wellbeing concerns.
  • Communicate effectively with clients, families, supervisors, and team members to ensure coordinated care.
  • Promote independence by encouraging client involvement in decision-making and daily routines.
  • Respond to client needs with empathy, patience, and adaptability.
  • Uphold confidentiality, privacy, and data protection requirements.
  • Follow organisational policies, procedures, and health and safety protocols to maintain a safe environment.

What We’re Looking For

  • Individuals aged 25–35. Preference for female applicants.
  • A manual driver’s licence is beneficial but not essential.
  • Full legal right to work in Australia.
  • Genuine passion for supporting people with disabilities and a commitment to person-centred practice.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport.
  • Patience, empathy, emotional resilience, and the ability to remain calm in challenging situations.
  • Physical fitness and capability to provide hands-on mobility support.
  • Reliability, punctuality, and flexibility to adapt to changing client needs.
  • Ability to maintain confidentiality and uphold client privacy.
  • Understanding of disability inclusion, rights-based practice, and safeguarding principles.
  • Certificate III in Disability Support (or equivalent) is preferred.
  • Current First Aid and CPR certification, or willingness to obtain shortly after commencement.
  • Satisfactory police clearance and Working with Children Check (or equivalent).
  • Experience in disability, aged care, or community services is highly valued but not essential.
  • Availability to commence ASAP.
