Poppy's Home Care is a nurse-led, family-owned provider dedicated to delivering caring support for older adults, enabling them to live safely, independently, and confidently in their homes. Our growth across the Peel and Rockingham regions is a testament to the trust placed in us by clients, families, allied health professionals, and our team. Primarily growing through referrals, we are expanding our Care Partner team and seek two experienced, client-focused individuals.

About the Role:

As a Care Partner, you will be instrumental in supporting older adults to maintain independence, wellbeing, choice, and dignity in their homes. You will collaborate with clients, families, Home Care Team Members, nurses, allied health professionals, and associated providers to deliver personalised, responsive, high-quality support aligned with individual goals and preferences. This is a relationship-focused role where you will genuinely connect with clients, advocate for their needs, and help them navigate their aged care journey.

Your responsibilities will include:

  • Building trusted, lasting relationships with clients and their families.
  • Conducting client visits and completing care planning, reviews, and reassessments.
  • Developing and maintaining person-centred care plans reflecting individual goals, preferences, and changing needs.
  • Supporting clients to maintain independence, confidence, safety, and quality of life.
  • Monitoring client wellbeing and responding proactively to changes in circumstances.
  • Coordinating services, referrals, equipment, and supports to ensure clients receive necessary care.
  • Collaborating with families, hospitals, allied health professionals, and associated providers for positive outcomes.
  • Maintaining accurate documentation, case notes, and care records.
  • Supporting clients to understand and maximise their Home Care Package or Support at Home funding.
  • Advocating for clients and ensuring their voice is central to all care decisions.
