About Hesse Rural Health Hesse is dedicated to advancing health and wellbeing by delivering best-practice health, aged care, and community services to support our community's wellbeing. Our staff are central to Hesse, with person-centred care for all stakeholders guiding our decision-making. As an innovative rural health service, Hesse provides acute, aged, and community-based services across parts of the Colac Otway, Golden Plains, and Surf Coast municipalities.

About the Role The Community Health Care Worker's role involves supporting Home Care Services recipients to maintain their independence. This is achieved through direct assistance with personal care, domestic duties, and community living tasks, all in accordance with agreed identified needs and services detailed in the care plan. Work will be conducted within the participant's home or community setting. Shifts will be based from either the Winchelsea or Bannockburn Hesse sites. The In Home Support Team Leader supervises this position, which operates within the Community Services program area.
