NDIS Support Coordinator - Orange Aboriginal Medical Service

Orange Aboriginal Medical Service (OAMS) is seeking a full-time NDIS Support Coordinator to join their expanding team in Orange. This role offers an opportunity to make a significant impact by empowering individuals with disability to achieve their objectives, exercise choice and control, and lead fulfilling lives.

About the Role

The NDIS Support Coordinator will collaborate with NDIS participants to implement their plans, establish connections with suitable supports, and develop their capacity for independent service management. This position is crucial in assisting participants navigate the NDIS and attain meaningful outcomes.

Key Responsibilities

  • Collaborate with participants to understand their NDIS plans, goals, needs, and preferences.
  • Assist participants in coordinating informal, mainstream, and funded supports.
  • Develop and maintain support plans, action plans, and service agreements.
  • Build and maintain strong relationships with service providers and community organisations.
  • Monitor the effectiveness of services and supports, addressing issues and changes as required.
  • Support participants through NDIS plan reviews, including preparing progress reports and goal evaluations.
  • Promote participant capacity building and independence.
  • Maintain accurate and compliant case notes and participant records.
  • Uphold the principles of dignity, respect, inclusion, and participant choice in all interactions.

About You

To be successful in this role, you will possess:

Essential Requirements

  • Sound knowledge of the NDIS and the role of Support Coordination.
  • Experience within disability services, community services, mental health, or a related field.
  • Strong understanding of person-centred and strengths-based practices.
  • Excellent communication, relationship-building, and interpersonal skills.
  • Demonstrated ability to manage a caseload and meet administrative requirements.
  • Strong organisational skills with the ability to work independently and manage competing priorities.
  • Current Driver’s Licence and access to a reliable vehicle (where travel is required).
  • Current NDIS Worker Screening Check or willingness to obtain.
  • Working With Children Check.

Requirements

The successful applicant will be required to:

  • Undertake and pass a Nationally Coordinated Criminal History Check.
  • Complete a pre-employment wellness check (including drug and alcohol screening).
  • Hold or obtain a NSW Working with Children Check.

Remuneration

SCHADS Award Level 3 Pay Point 1 $40.49 per hour + superannuation.
