NDIS Support Worker - All Over Perth Metro

Aastha Community Services is seeking energetic, passionate, and highly committed support workers to join our expanding team.

In this role, you will assist individuals living with a disability to participate and engage in social and community activities, as well as in residential facilities for supported independent living. You will support NDIS participants with empathy, humanity, and care.

Responsibilities and Duties:

  • Provide support to participants aligned with Aastha's social model of recovery.
  • Offer support to participants, assisting them to engage in lifestyle activities of their choice, meet daily living needs, and achieve greater social participation within their community.
  • Deliver short-term interventions, education, support, and mentoring to participants.
  • Support and assist participants in achieving their goals and needs as outlined in their approved NDIS plan.
  • Deliver high-quality, person-centred services to meet participant needs.
  • Provide weekday and weekend support as required by participants.

Qualifications and Skills:

To be considered for this position, you must possess:

  • A full WA Driver's License.
  • A current Working With Children's Check.
  • A current valid First Aid Certificate.
  • A current National Police Check.
  • Your own vehicle with full, comprehensive roadworthy vehicle insurance.
  • An understanding of the National Disability Insurance Scheme (NDIS) and the Disability Sector.
  • An understanding of how to manage a caseload autonomously.
  • The ability to organise your own timetable/roster and meet deadlines/appointments.
  • The ability to write case notes articulately, clearly, and concisely.
