Job Summary: We are seeking a compassionate, organised and proactive Disability Service Coordinator to join our team. In this role, you’ll support NDIS participants by developing tailored plans, coordinating schedules, and ensuring services are delivered safely and to a high standard. You’ll act as the main contact for clients, attend stakeholder meetings and oversee mentor support. This position is ideal for someone who builds strong relationships, stays on top of documentation and compliance, and is committed to delivering high‑quality, person‑centred support.

This is a full-time position, however part-time hours will be considered for the right applicant.

Key Responsibilities:

  • Develop individualised support plans and risk assessments that reflect each participant’s needs, goals and circumstances.
  • Build strong, trusting relationships with participants, families, caregivers and mentors to support collaboration and positive outcomes.
  • Manage a caseload of up to 50 participants, providing responsive, person‑centred coordination.
  • Be the primary point of contact for your clients and their mentors, ensuring high quality and timely communication and support.
  • Coordinate and manage client schedules, including mentor matching, rostering, service changes, and continuity of support.
  • Manage client funding throughout the plan period, ensuring supports are appropriately allocated, compliant and sustainable.
  • Attend client and stakeholder meetings, prepare high‑quality progress reports, notes and documentation for internal and external stakeholders.
  • Monitor the effectiveness of supports and adjust plans, strategies and risk controls as required.
  • Manage incidents and complaints in line with organisational procedures and legislative requirements.
  • Maintain up‑to‑date knowledge of NDIS requirements, disability legislation, sector programs and funding structures.
  • Support recruitment by identifying mentor requirements for specific client needs and assisting with selection and onboarding.

Qualifications and Experience:

  • Bachelor’s degree in social work, Psychology, Disability Studies, or a related field (preferred).
  • Relevant experience in case management, advocacy, or disability service coordination.
  • Experience within the NDIS, disability, community services, aged care or health sectors is highly regarded.
  • Strong interpersonal and communication skills.
  • Compassionate and person-centred approach.
  • Excellent problem-solving and organisational skills.
  • Ability to manage multiple priorities and adapt to changing needs.
  • Proficiency in Microsoft Office and client management systems.
  • Experience preparing reports, risk assessments and compliance documentation.
  • Current National Police Clearance and Working with Children Check (or willingness to obtain).
  • Current Driver’s Licence and vehicle.
