Qualifications & Experience:

• Certificate or Diploma qualification or equivalent in Care (Desired but not required).
• NDIS Workers Screening Clearance (Or willing to obtain).
• Working with Children Check (Or willing to obtain).
• Criminal Record Check (Or willing to obtain).
• Experience in Case Management and/or relevant Case Management Qualifications (Desired but not required).

Tasks & Responsibilities:

• Prepare and distribute schedules for customer services.
• Manage rostering, including arranging replacement staff for planned or unplanned absences and shift changes.
• Undertake Stakeholder Management, taking ownership for developing, retaining, and maintaining these networks.
• Respond to enquiries regarding current and potential NDIS participants.
• Assist and support a team of Disability Support Workers.
• Contribute to the development, implementation, monitoring, and review of care plans to meet client needs and enhance quality of life.
• Promote a consultative and collaborative approach to client care planning and quality management activities.
• Conduct comprehensive and accurate client assessments.
• Maintain appropriate records and documentation.
• Complete all necessary administration to the required standard, ensuring full compliance.
• Fulfil other duties as required by management and other department personnel.
• After-hours and on-call duties will be required, with appropriate award and standby rates applying.
• Occasional client transport may be necessary.
