Western District Health Service seeks a passionate individual to support older people in maintaining independent lives at home. As a Support at Home – Case Manager in Hamilton, you will provide high-quality, person-centred care management, contributing positively to your community daily. This position is suited for an individual who excels at relationship building, problem-solving, and coordinating services to enhance the quality of life, dignity, and choices for older adults.

Key Responsibilities:

  • Person-Centred Care: Deliver tailored care management focused on supporting independence, dignity, and individual choice for older individuals residing at home.
  • Care Planning & Coordination: Develop, implement, and review personalised care plans in collaboration with participants, their families, and service providers.
  • Stakeholder Engagement: Liaise effectively with clients, families, healthcare professionals, and external service providers to ensure coordinated and seamless care delivery.
  • Budget & Program Management: Oversee individual program budgets, ensuring the effective delivery of supports within allocated funding guidelines.
  • Monitoring & Review: Conduct regular assessments of client outcomes and make necessary adjustments to care plans to address evolving needs.
  • Documentation & Compliance: Maintain accurate records and ensure adherence to program requirements, standards, and reporting obligations.
  • Teamwork & Collaboration: Function both independently and as part of a multidisciplinary team within a dynamic and supportive setting.

What You Bring:

  • Experience in aged care case management.
  • Demonstrated ability to manage budgets and funding programs.
  • Strong communication and relationship-building skills.
  • Capacity to coordinate complex supports within a community environment.
  • Excellent organisational and time management skills.
  • A commitment to person-centred care and achieving positive client outcomes.
  • The ability to work both independently and as part of a multidisciplinary team.
