Support Coordinator

Curamoir Healthcare Recruitment PTY LTD is seeking a full-time, onsite Support Coordinator in Canberra ACT.

About the Role This position offers a salary of $95,977 plus Super and a car allowance. You will join a multidisciplinary team dedicated to making a difference within the disabilities sector. The client is a values-driven, registered NDIS provider offering person-centred disability support services. These services include Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), in-home support, support coordination, and community participation.

As a Support Coordinator, your primary responsibility will be to provide varying levels of coordination to NDIS participants. The aim is to empower individuals to build their capacity and that of their networks, enabling them to independently manage their challenges and supports, achieve their goals, and exercise choice in obtaining services that align with their chosen lifestyles.

Responsibilities and Duties:

  • Manage a caseload of Support Coordination clients as agreed with the supervisor, achieving claimable hours as discussed and agreed, and providing case management services.
  • Participate in special projects as required.
  • Assist NDIS participants with their plan implementation.
  • Operate within the funding levels allocated to each participant.
  • Foster effective working relationships and communication with carers, family members, external services, and other key stakeholders.
  • Communicate effectively with diverse individuals within the organisation and the broader community.
  • Connect clients with service providers to facilitate the achievement of their NDIS goals.
  • Continuously gather and document relevant participant information from other services to develop evidence-based reports and updates.
  • Support participants in preparing for plan review meetings and in building their capacity for independent living.
  • Empower clients to exercise choice and control in accessing a range of formal and informal mainstream and community-based supports, identifying opportunities for innovative service delivery.
  • Consistently seek opportunities to build the capacity of clients and their families, promoting greater independence.
  • Conduct risk assessments and develop support plans and shift plans for participants.
  • Adhere to organisational policies and procedures, and funding guidelines.
  • Comply with Occupational Health and Safety standards, guidelines, and practices.

Your Skills:

  • A relevant tertiary qualification.
  • 1 to 2 years of experience as an NDIS Support Coordinator.
  • A Working with Vulnerable People Card and a current Police Clearance Certificate (obtained within the last three months).
  • Demonstrated ability to work independently and collaboratively to motivate, support, and communicate effectively with individuals at risk of harm and/or exhibiting challenging behaviours.
  • A current driver's licence and a car with comprehensive care insurance.
