Support Worker – Relight Your Care Pty Ltd

Relight Your Care Pty Ltd is seeking a casual Support Worker to join our team in Geelong. We are dedicated to empowering individuals to lead fulfilling, independent lives through holistic, personalised in-home and community support. About the Role As a Support Worker, you will be instrumental in assisting participants to achieve their personal goals, build independence, and engage within their community. This role involves providing personalised in-home and community-based support aimed at improving daily living, employment, social, and recreational skills.

Your responsibilities will include:

  • Supporting clients with daily tasks, employment, social, and recreational activities.
  • Facilitating community access and encouraging participation in activities and outings.
  • Providing person-centred direct care in accordance with individual service plans.
  • Monitoring client wellbeing, safety, and progress.
  • Maintaining accurate documentation and communication notes.
  • Collaborating with families, carers, case managers, and allied health professionals.
  • Adhering to all work health, safety, medication, and privacy guidelines.
  • Participating in regular team meetings, supervision, and training opportunities.

Qualifications and Experience

  • Certificate III or IV in Disability, Aged Care, or Individual Support is preferred.
  • A Mental Health Certificate is preferred.
  • At least one year of hands-on experience in personal care is preferred.
  • Experience supporting children or individuals with mental health disabilities is preferred.

Other Requirements

  • Understanding of NDIS principles and relevant legislation.
  • Current First Aid and CPR certification.
  • Completion of NDIS Worker Orientation and supporting practice modules.
  • A Police Check, NDIS Worker Screening Check, and Working with Children Check.
  • Up-to-date Covid-19 vaccination.
  • A current driver’s licence and fully insured vehicle.
  • Full Australian working rights.
